Devolution focus:

One of our strategic objectives is to improve the health and wellbeing of all of the residents of
Greater Manchester from early age to older people, recognising that this will only be
achieved with a focus on the prevention of ill health and the promotion of wellbeing.

Devolution will help organisations to formally work together to coordinate services in new
ways to tackle some of the major health challenges we face in our region.


  • Hospital admissions for people with lung disease have decreased by 12 per cent after
    the introduction of this scheme
  • Service users have also reported significant improvements in their health, increased
    confidence and independence and a better service experience overall.


The Rochdale borough COPD Response and Pulmonary Rehabilitation Team, part of
Pennine Care NHS Foundation Trust, is made up of many different specialists who provide
therapy, treatment and support to people who have a range of lung conditions.

A range of disciplines contribute to the success of the team by ensuring that care can be tailored to
the needs of each individual.

The team completed a training programme around supporting service users to self-manage their condition.

This helps to ensure they can enjoy the best quality of life possible and reduce the risk of their symptoms
becoming worse – meaning they are less likely to require hospital care.

One service user who has reaped the benefits is 69-year-old Christine Power from Castleton, who has experienced COPD since her 40s.

After being discharged from hospital, she received regular visits and phone support from the team who advised her on how to prevent her symptoms worsening and warning signs to look out for.

She said: “The support provided by the team enabled me to stay at home and made me feel safer and more confident. They’re always there if I need support which is reassuring as I live on my own.”
